افتراقپذیری بازشناسی و سوگیری پاسخ در تست یادگیری کلامی شیراز میان سالمندان با اختلال شناختی خفیف، بیماری آلزایمر و حافظه نرمال

<div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Tahoma;"><span style="line-height:2;"><strong>اهداف </strong>بیشتر مطالعات تأثیر پیری پاتولوژیکی مغز بر یادآوری اطلاعات و عملکرد حافظه بازشناسی بیماران (با استفاده از روش بله / خیر) را مورد بررسی قرار میدهند و این باعث میشود تصویر ناکاملی از نقص عملکرد حافظه ارائه داده شود و دیگر عوامل مربوط به حافظه بازشناسی مثل افتراقپذیری و سوگیری از پاسخ، مورد توجه قرار نگیرد. به همین خاطر، ما در این مطالعه تأثیر اختلال شناختی خفیف و بیماری آلزایمر بر روی زیرمقیاسهای حافظه بازشناسی، یعنی شاخصهای افتراقپذیری بازشناسی و سوگیری پاسخ را در تست یادگیری کلامی شیراز مورد بررسی قرار دادیم.<br>
<strong>مواد و روش ها</strong> این مطالعه از نوع توصیفیمقطعی بود که در سال 1400 بر روی 190 نفر از سالمندان 60 تا 89 سال شهر شیراز که معیارهای ورود به مطالعه را داشتند، انجام شد. ابزارهای جمعآوری اطلاعات شامل 2 تست یادگیری کلامی شیراز و تست کوتاه وضعیت ذهنی بود. تجزیهوتحلیل آماری متغیرها با استفاده از آزمونهای توصیفی (فراوانی، میانگین و انحراف معیار) و تحلیلی (آزمون آنووای یکراهه و آزمون تعقیبی شفه) و در سطح معناداری ( 0/05>P) با کمک نرمافزار SPSS (نسخه 19) انجام شد.<br>
<strong>یافته ها</strong> نتایج نشان داد بیماران مبتلا به آلزایمر و اختلال شناختی خفیف آمنزیا نسبت به سالمندان سالم در عملکرد حافظه بازشناسی بهطور معناداری نمرههای پایینتری گرفتند (0/001>P). همچنین در شاخصهای افتراقپذیری بازشناسی (کل، معنایی، منبع و جدید) گروه سالمندان مبتلا به اختلال شناختی خفیف آمنزیا و سالمندان مبتلا به آلزایمر نسبت به سالمندان سالم، خطاهای مثبت کاذب بیشتر و عملکرد ضعیفتری داشتند (0/001>P). اما نتایج سوگیری از پاسخ نشان داد سالمندان مبتلا به آلزایمر بهطور معناداری سوگیری پاسخ بالاتری نسبت به سالمندان سالم و سالمندان با اختلال شناختی خفیف آمنزیا داشتند (0/001>P).<br>
<strong>نتیجه گیری</strong> یافتههای پژوهش حاضر پیشنهاد میدهد شناسایی ذات و الگوهای تغییر شناختی حافظه بازشناسی (افتراقپذیری بازشناسی) در بیماران مبتلا به اختلال شناختی خفیف میتواند اطلاعات سودمندی باشد که در مرحله پریدومال به تشخیص تبدیل این اختلال به بیماری آلزایمر کمک میکند.</span></span></span></div>
<div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Tahoma;"><span style="line-height:2;"><strong>Objectives </strong>Most studies have investigated the effect of pathological aging brain on information recall and recognition memory of patients by using yes-no procedures. For this reason, they provide an incomplete picture of memory defect, and other factors related to recognition memory such as recognition discriminability (RD) and response bias (RB) are not taken into consideration. In this regard, present study aims to examine the effect of amnestic mild cognitive impairment (MCI) and Alzheimer’s disease on RD and RB using the Shiraz verbal learning test (SVLT) compared to healthy peers.<br>
<strong>Methods & Materials </strong>This descriptive cross-sectional study was conducted on 190 eligible older people aged 60-89 years with amnestic MCI, Alzheimer’s disease, and normal memory in Shiraz, Iran, in 2021. The data collection tools were SVLT and mini-mental status examination. Data analysis was done in SPSS software, version 19 using descriptive statistics (No., Mean±SD) and analytical tests (ANOVA and Scheffe’s test). The significance level was set at 0.05.<br>
<strong>Results </strong>The patients with amnestic MCI and Alzheimer’s disease had significantly lower scores than the healthy older people in recognition memory performance (P<0.001). They also had lower scores and more false-positive errors in RD domains (global, semantic, source, and new) (P<0.001). in RB, the patients with Alzheimer’s disease had significantly higher scores than older people with amnestic MCI and normal memory (P<0.001). <br>
<strong>Conclusion </strong>The study of recognition memory performance (RB and RD) in patients with amnestic MCI can provide useful information to predict Alzheimer’s disease in the prodromal stage.<br>
