بررسی رابطه سن با میزان گلبولهای سفید خون محیطی در بیماران مبتلا به عفونت منتشر در دو بیمارستان تهران

<p dir="RTL" style="text-align: justify;"><strong>اهداف:</strong> مقدار طبیعی <span dir="LTR">WBC</span> در افراد مسن مختصری کاهش یافته ولی در <span dir="LTR">Sepsis</span> میزان آن افزایش می یابد. لکوپنی و لکوسیتوز به عنوان ریسک فاکتورهای همراه با باکتریمی در مورتالیتی افراد مسن نقش دارد. این تحقیق جهت شناخت رابطه سن با تغییرات <span dir="LTR">WBC</span> بیماران مبتلا به <span dir="LTR">Sepsis</span> انجام گرفت.</p>
<p dir="RTL" style="text-align: justify;"><strong>مواد و روشها:</strong> روش تحقیق مورد شاهدی بوده که روی بیماران مبتلا به <span dir="LTR">Sepsis</span> در بیمارستان های لقمان حکیم و بوعلی تهران در سال های 1385-1380 انجام گرفت.</p>
<p dir="RTL" style="text-align: justify;"><strong>یافتهها:</strong> 130 بیمار در دو گروه 65 ساله و بالاتر (مورد) و 64 ساله و جوانتر (شاهد) بررسی شدند. میانگین گلبول سفید خون بیماران گروه مورد 1424/2±17061/54 و در گروه شاهد 9888±13567/7 بوده که بین میانگین <span dir="LTR">WBC</span> در دو گروه اختلاف معنیدار آماری مشاهده نشد. ولی در میانگین تعداد نبض و درجه حرارت دهانی بیماران مسن و جوان اختلاف معنی دار بود (<span dir="LTR">p<0/05</span>) در بیماران مسن بین سن با سابقه عفونت اخیر و نیز سابقه بستری در یک ماه گذشته و همچنین بین منشا احتمالی عفونت و <span dir="LTR">WBC</span> رابطه معنی دار آماری وجود داشت (<span dir="LTR">p<0/05</span>). در هیچ یک از دو گروه بین مرگ با <span dir="LTR">WBC</span> و نیز مرگ با منشا احتمالی عفونت رابطه معنی دار وجود نداشت.</p>
<p dir="RTL" style="text-align: justify;"><strong>نتیجهگیری:</strong> بین <span dir="LTR">WBC</span> و سن در بیماران مسن رابطه معنی دار آماری مشاهده نشد. <span dir="LTR">Sepsis</span> در افراد مسن می تواند بدون تب و تعداد نبض در دقیقه کمتری تظاهر نموده و سابقه عفونت و همچنین سابقه بستری در بیمارستان در یک ماه اخیر در ارزیابی این بیماران اهمیت دارد.</p>
<p style="text-align: justify;"><strong>Objectives: </strong>In the elderly, total WBc decreases slightly. in response to sepsis the increase in WBc is so dramatic. There are some reports about effects of increasing the number of WBc as a predisposing factor of bacteremia and further association of neutrophili a and leukopenia with increased mortality rate in the elderly. The aim of this study was to determine relationship between age and peripheral WBc counts in young versus elderly patients with sepsis<span dir="RTL">.</span></p>
<p style="text-align: justify;"><strong>Methods & Materials:</strong> A case-control study was designed on 65 admitted septic patients over 65 years of age as cases and 65 admitted septic patients less than65 years of age as controls according to Loghman Hakim and Booali Hospitals, during 2001- 2006<span dir="RTL">.</span></p>
<p style="text-align: justify;"><strong>Results:</strong> Mean WBc was 17061/54<span style="color: rgb(0, 0, 0); font-family: Symbol; font-size: 13.3333px; text-align: justify;">±</span>1424/2 in the cases and 13567/7<span style="color: rgb(0, 0, 0); font-family: Symbol; font-size: 13.3333px; text-align: justify;">±</span>9888 in the controls. There was not a significant difference between mean of WBe. There was a significant difference between mean of pulse rate, and oral temperature in two groups. (P<0.05). In elderly, there was a significant correlation between age with history of infection and age with history of hospitalization during last month and also a significant correlation between age with source of probable infection ((P<0.05), but there was no a significant correlation between death with WBc and death with source of probable infection<span dir="RTL">.</span></p>
<p style="text-align: justify;"><strong>Conclusion:</strong> Bacteremia in elderly can be afebrile and with pulse rate less than otber patients. The history of infection and history of hospitalization during last month in elderly with sepsis are considered to be important in evaluation<span dir="RTL">.</span></p>
