Extended Abstract
1. Objectives

The increasing number of elderly people and the prevalence of physical disabilities in them have drawn the attention of many health and social care systems [1]. In this regard, it is important to study the reduced ability for postural control which is one of the most important and most common problems of the elderly [2, 3]. In general, balance is defined as the ability of the body to maintain the center of gravity within the limits of stability [4]. 
Studies have reported that participation in aquatic exercises to increase the proprioceptive acuity at lower limb joints can play an important role in improving balance in the elderly [5]. However, it has not yet been determined in which lower limb joints the proprioceptive acuity play the most role. In this regard, this study examined and compared the role of the proprioceptive acuity at ankle and knee in improving the elderly balance after aquatic exercise training.
2. Methods and Materials
This study is a clinical trial (code: IRCT2016031227022N1). Study population consisted of 80 elderly people living in Kerman City, Iran. Of them, 30 volunteers were randomly selected and divided into experimental (n=15) and control (n=15) group. The inclusion criteria were being male, 60-70 years old and able to walk independently. Subjects who had previous damage or surgery, especially in the lower limbs as well as those with a history of chronic diseases, use of psychotropic medications or participating in a particular sport on a regular basis were excluded from the study. 
In order to measure the balance and stability of the subjects, Biodex balance system (ICC≥0.79) was used. Measurement of proprioceptive acuity at ankle and knee joints was performed by photography method (ICC≥0.97). The experimental group intervened by a standard aquatic exercise training protocol for 8 weeks, 3 sessions a week. Each training session consisted of 5 minutes of warm-up, 35 minutes of workout and 5 minutes of cooling down. The control group did not participate in any particular physical activity, and only did their daily routine activities. After 8-week aquatic exercise training, all subjects were reassessed and different stages of measurement were repeated for them. Finally, the collected data were analyzed using Shapiro–Wilk test, Levene's test, Independent t test, combined ANOVA and Pearson correlation test (α=0.05) in SPSS V. 21.
3. Result
To determine the homogeneity of experimental and control groups in this study, the Independent t test was used. Results showed no significant difference between groups in terms of age, height, weight and body mass index, and both groups were homogeneous in these respects (P≥0.05). Independent t test was also used to compare the postural control ability as well as proprioceptive acuity between the two groups (P≥0.05). The results showed no significant difference between them and the groups were homogeneous (P≥0.05). In order to evaluate the effect of 8-week aquatic exercise training on the elderly balance, combined ANOVA was applied. 
Its results indicated that a significant interaction between time (pretest-posttest) and group (control-experimental) in relation to the balance indices (P=0.015). It was also found that the main effect of time and group on the balance indices of the subjects was significant (P=0.001 and P=0.007, respectively). Results of proprioceptive acuity changes before and after intervention are shown in Figure 1.
The Pearson correlation test was used to examine the correlation between proprioceptive acuity and postural control ability in the elderly. In this regard, results showed a significant correlation between the changes in ankle joint proprioceptive acuity and the ability for postural control after intervention (P=0.031, and 0.045 for dominant and non-dominant ankles, respectively), while no significant correlation was observed between the changes in knee joint proprioceptive acuity and the postural control ability (P=0.136, and 0.760 for dominant and non-dominant knees, respectively).
4. Conclusion
Considering the relationship between ankle joint proprioceptive acuity and the ability for postural control in the elderly men after 8-week aquatic exercise training, it seems that increased accuracy in the proprioceptive acuity of ankle joint is more effective in reducing the oscillations of the elderly's center of gravity compared to the knee joint proprioceptive acuity. As a result, it is recommended that, in training programs for improving the postural control ability of the elderly, the special role of the ankle joint in improving this ability should be taken to into consideration.

مقدمه
افزایش روزافزون جمعیت سالمندان که با شیوع ناتوانی‌های جسمی در این افراد همراه بوده است، توجه بسیاری از سیستم‌های مراقبتی، بهداشتی و اجتماعی را به خود معطوف کرده است [1]. در این بین توجه به کاهش توانایی کنترل پاسچر و تعادل سالمندان اهمیت ویژه‌ای دارد، زیرا این عامل از جمله مهم‌ترین و شایع‌ترین مشکلات جسمانی سالمندان است [2]. با ورود به دوره سالمندی تغییراتی در عملکرد سیستم‌های اسکلتی‌عضلانی، دهلیزی، حسی‌پیکری و همچنین بینایی سالمندان رخ می دهد و از آنجا که کنترل تعادل به مشارکت در سه حیطه پردازش اطلاعات به کمک حواس، یکپارچگی مرکزی و همچنین پاسخ حرکتی به محرک‌ها نیازمند است، هرگونه نقص در این سیستم‌ها می‌تواند فرد را در وضعیت ازدست‌دادن تعادل و زمین‌خوردن قرار دهد [3]. 
تعادل و کنترل پاسچر به عنوان توانایی نگهداری مرکز ثقل بدن درون سطح اتکا شناخته می‌شود که می‌تواند نقش مهمی در پیشگیری از زمین‌خوردن افراد هنگام راه‌رفتن (تعادل پویا) و ایستادن (تعادل ایستا) ایفا کند [4]. عوامل مختلفی در ایجاد تعادل و بهبود توانایی کنترل پاسچر نقش دارند که بر اساس تحقیقات پیشین، به نظر می‌رسد میزان دقت حس عمقی در مفاصل اندام تحتانی یکی از مهم ترین عوامل مؤثر در این زمینه و از اجزای مؤثر در راه رفتن به شمار می‌رود [5]. 
اعتقاد بر آن است افرادی که با کاهش دقت حس عمقی در اندام تحتانی خود مواجهند، قادر به تشخیص به‌موقع نوسانات بدن نیستند و نمی‌توانند نیروی پایدارکننده مناسب برای حفظ تعادل بدن را به‌خوبی ایجاد کنند [6]. هن و همکاران و همچنین شیم و همکاران طی تحقیقاتی گزارش کردند کاهش دقت حس عمقی در اندام تحتانی ارتباط مستقیمی با افزایش ریسک زمین‌خوردن آزمودنی‌ها در هنگام ایستادن یا راه رفتن دارد [8 ،7]. گزارش‌ها همچنین حاکی از آن است که تغییرات وابسته به سن در سالمندان که به تغییراتی در سیستم عصبی این افراد منجر می‌شود، می‌تواند عملکرد حس عمقی آن‌ها را نیز به طرز چشمگیری کاهش دهد [9]. با توجه به اینکه گفته شد حس عمقی از عوامل اصلی درگیر در تعادل افراد به شمار می‌رود، کاهش در توانایی عملکردی آن طی دوره سالمندی می‌تواند قابلیت کنترل پاسچر و توانایی پیشگیری از زمین‌خوردن این افراد را به‌مراتب بیشتر از قبل با محدودیت مواجه کند [6]. 
با توجه به مشکلات برشمرده، تاکنون تحقیقات بسیاری در راستای بهبود توانایی‌های سالمندان صورت گرفته است که نتایج آن‌ها عمدتاً حاکی از نقش مؤثر تمرینات آبی در افزایش قابلیت‌های جسمانی و همچنین پیشگیری، به تأخیر انداختن و درمان مشکلات ناشی از فرایند پیری است [11 ،10]. اعتقاد بر آن است که تمرین در آب محیطی را فراهم می‌کند که ضمن کاهش ترس از اجرای حرکات و زمین‌خوردن‌های احتمالی، انتقال ورودی‌های اعصاب‌آوران را نیز افزایش می‌دهد و با بهبود دقت حس عمقی در مفاصل اندام تحتانی امکان برانگیختگی عضلات و حفظ تعادل بدن را با سهولت بیشتری فراهم می‌کند [5]. 
با وجود اینکه نقش تمرینات آبی در بهبود حس عمقی اندام تحتانی انکارناپذیر به نظر می‌رسد و این موضوع در تحقیقات پیشین به‌خوبی نشان داده شده است، هنوز این ابهام وجود دارد که پس از مشارکت در تمرینات آبی، تغییرات حس عمقی در کدامیک از مفاصل اندام تحتانی بیشترین نقش را در بهبود تعادل سالمندان دارد؟ با توجه به اینکه شناسایی چنین ارتباطی می‌تواند نقش مهمی در برنامه‌ریزی‌های آتی برای پیشگیری از زمین‌خوردن سالمندان و استفاده حداکثری از برنامه‌های تمرینی داشته باشد، هدف از تحقیق حاضر بررسی نقش حس عمقی مچ پا و زانو در بهبود تعادل سالمندان پس از یک دوره تمرینات آبی است.
روش مطالعه
تحقیق حاضر از نوع کارآزمایی بالینی (با کد IRCT2016031227022N1) است. جامعه آماری آن را 80 نفر از سالمندان شهر کرمان تشکیل می‌دادند. بدین ترتیب از بین افراد داوطلب 30 نفر به صورت تصادفی انتخاب و به دو گروه تجربی (15 نفر) و کنترل (15 نفر) تقسیم شدند. معیارهای انتخاب آزمودنی‌ها در تحقیق حاضر شامل مردبودن، قرارداشتن در محدوده سنی 60 تا 70 سال و همچنین توانایی راه‌رفتن به صورت مستقل بود. در ادامه نیز آزمودنی‌هایی که آسیب قبلی یا عمل جراحی به ویژه در اندام تحتانی داشتند و همچنین افرادی که سابقه بیماری‌های مزمن، استفاده از داروهای عصبی یا مشارکت در ورزش خاص به طور منظم را داشتند، از تحقیق کنار گذاشته شدند [13 ،12]. 
غربالگری افراد با استفاده از پرسش‌نامه تاریخچه پزشکی که قبل از مطالعه تکمیل شده بود، انجام گرفت. برای تعیین آمادگی افراد شرکت‌کننده در دوره تمرینی از پرسش‌نامه آمادگی فعالیت جسمانی استفاده شد. افراد واجد شرایط بر اساس معیارهای مذکور انتخاب شدند و پس از تکمیل فرم رضایت‌نامه به عنوان آزمودنی‌های تحقیق بررسی شدند. 
اندازه‌گیری نوسانات قامتی آزمودنی‌ها در تحقیق حاضر با استفاده از دستگاه تعادلی بایودکس (0/79≤ICC) صورت گرفت [14]. بدین ترتیب از آزمودنی‌ها خواسته شد تا ضمن قرارگرفتن با پای برهنه روی دستگاه بایودکس و با فاصله یک عرض شانه بین پاهایشان تلاش کنند مرکز ثقل را با مرکز صفحه بایودکس منطبق کنند و نشانگر متحرک را به نقطه وسط صفحه برسانند. در ادامه، پس از اینکه هریک از آزمودنی‌ها یک‌مرتبه آزمون را به صورت آزمایشی اجرا کردند و با نحوه کامل اجرای آن آشنا شدند، آزمون اصلی انجام می‌شد. درجه ناپایداری سطح اتکا در حین اجرای آزمون باید روی سطح 8 تنظیم می‌شد و مدت زمان اجرای هر آزمون نیز 20 ثانیه (3 تکرار با فواصل استراحت 10 ثانیه‌ای بین تکرارها) در نظر گرفته می‌شد. 
در ادامه برای اندازه‌گیری دقت حس عمقی در مفاصل مچ پا و زانو از روش عکس‌برداری (0/97≤ICC) استفاده شد [17-15]. برای این منظور و پس از آنکه مارکرگذاری پوستی در سمت خارجی اندام آزموده شد [18]، هر کدام از آزمودنی‌ها روی صندلی می‌نشستند و پاها را در وضعیت آویزان قرار می‌دادند. سپس از آزمودنی خواسته می‌شد تا ابتدا با چشمان باز مفصل موردآزمون را تا زاویه هدف حرکت دهد و در این وضعیت اولین عکس از جانب خارجی مفصل گرفته می‌شد [15]. 
در ادامه برای حذف مداخله بینایی در حین اندازه‌گیری از چشم‌بند استفاده و از آزمودنی خواسته شد که زاویه هدف را مجدداً بازسازی کند. این عمل سه بار تکرار و با هر بار تکرار، از زاویه بازسازی‌شده عکس‌برداری صورت می گرفت تا میانگین این سه زاویه به عنوان رکورد آزمودنی در بازسازی زاویه هدف ثبت شود [18 ،15]. در نهایت با نرم‌افزار اتوکد 2014 تمامی تصاویر ثبت‌شده تحلیل شد و میزان اختلاف موجود بین زاویه بازسازی‌شده توسط آزمودنی با زاویه هدف (وضعیت چشم باز)، بدون در نظر گرفتن مثبت یا منفی‌بودن جهت حرکت به عنوان زاویه خطا (خطای مطلق) ثبت شد [15]. در مفصل مچ پا از زاویه 10 درجه دورسی فلکشن و در مفصل زانو از زاویه 45 درجه فلکشن به عنوان زاویه هدف استفاده شد [19].
با اتمام پیش‌آزمون و بعد از فراهم‌کردن مقدمات کار، گروه تجربی به مدت هشت هفته و به صورت سه جلسه در هفته به اجرای برنامه تمرینات آبی استاندارد پرداختند. هر جلسه تمرینی شامل پنج دقیقه گرم‌کردن، 35 دقیقه تمرین و پنج دقیقه بازگشت به حال اولیه بود. تمامی تمرینات باید در عمقی صورت می‌گرفت که آب در ارتفاع بین کمر و جناغ سینه آزمودنی‌ها قرار داشته باشد. تمام فعالیت‌ها در حین هشت هفته به دلیل دستکاری و تغییر در وضعیت دست‌ها مانند باز و بسته‌کردن دست‌ها حین حفظ تعادل یا به دلیل افزایش سختی فعالیت‌های انجام‌شده مانند افزایش مسافت یا مدت قدم‌زدن و طول گام‌ها و یا بستن چشم‌ها حین ایستادن، به طور فزاینده‌ای مشکل‌تر می‌شدند. در تمام مدت اجرای تحقیق، آزمودنی‌های گروه کنترل در هیچ فعالیت خاص ورزشی شرکت نمی‌کردند و تنها به اجرای فعالیت‌های عادی روزمره خود می‌پرداختند. پس از اتمام هشت هفته برنامه تمرینات آبی برای گروه تجربی، تمامی آزمودنی‌ها از نظر متغیرهای تحقیق دوباره ارزیابی شدند و مراحل مختلف اندازه‌گیری مانند آنچه در پیش‌آزمون شرح داده شد، مجدداً اجرا شد. 
در نهایت، اطلاعات خام به‌دست‌آمده از متغیرهای تحقیق با استفاده از نرم‌افزار SPSS نسخه 21 تحلیل شدند. بدین ترتیب پس از تأیید عادی‌بودن داده‌ها و همچنین بررسی همگنی واریانس‌ها به ترتیب با استفاده از آزمون شاپیرو-ویک و آزمون لونز، برای مقایسه میانگین اطلاعات پیش‌آزمون و پس‌آزمونِ گروه‌های سالمند از آزمون تحلیل واریانس ترکیبی استفاده شد. در ادامه، برای بررسی ارتباط بین متغیرهای دقت حس عمقی و تعادل از آزمون همبستگی پیرسون استفاده شد. سطح معنی‌داری تحقیق حاضر برابر با 95 درصد با آلفای کوچک‌تر یا مساوی با 0/05در نظر گرفته شد. 
یافته‌ها
اطلاعات جمعیت شناختی آزمودنی‌ها به تفکیک گروه مربوطه در جدول شماره 1 گزارش شده است. برای تعیین همگن‌بودن گروه‌ها در این متغیرها از آزمون تی مستقل استفاده شد که نتایج این آزمون نیز در جدول شماره 1 ارائه شده است و نشان می‌دهد بین سن، قد، وزن و شاخص توده بدنی گروه‌ها اختلاف معنی‌داری وجود ندارد و گروه‌ها در این متغیرها همگن هستند (P>0/05). در ادامه برای مقایسه توانایی کنترل پاسچر و همچنین دقت حس عمقی آزمودنی‌های گروه کنترل و تجربی نیز از آزمون تی مستقل استفاده شد که نتایج نشان داد بین متغیرهای مذکور در دو گروه اختلاف معنی‌داری وجود ندارد و گروه‌ها در این متغیرها نیز همگن هستند (P>0/05).
برای بررسی تأثیر هشت هفته تمرینات آبی بر نوسانات قامتی آزمودنی‌ها از آزمون تحلیل واریانس ترکیبی استفاده شد. بر این اساس مشخص شد اثر متقابل معنی‌داری بین زمان (پیش‌آزمون، پس‌آزمون) و گروه (کنترل و تجربی) در ارتباط با شاخص‌های تعادلی آزمودنی‌ها وجود دارد (P<0/05). همچنین مشخص شد اثر اصلی زمان و اثر اصلی گروه در ارتباط با شاخص‌های تعادلی آزمودنی‌ها معنی‌دار است (جدول شماره 2). اطلاعات مربوط به دقت حس عمقی مفاصل آزمودنی‌ها و میزان تغییرات آن‌ها از پیش‌آزمون تا پس‌آزمون نیز در تصویر شماره 1 ارائه شده است.
برای بررسی ارتباط بین میزان تغییرات دقت حس عمقی و توانایی کنترل پاسچر سالمندان از آزمون همبستگی پیرسون استفاده شد (جدول شماره 3). بر این اساس مشخص شد متعاقب هشت هفته تمرینات آبی، بین تغییرات حس عمقی مچ پا و بهبود توانایی کنترل پاسچر سالمندان همبستگی معنی‌داری وجود دارد (0/05>P).
بحث
نتایج به‌دست‌آمده از تحقیق حاضر نشان داد اجرای هشت هفته تمرینات آبی می‌تواند توانایی کنترل پاسچر سالمندان را به شکل معنی‌داری بهبود ببخشد (جدول شماره 2)؛ این یافته‌ها هم‌راستا با اغلب گزارش‌های پیشین در این زمینه است که معتقدند اجرای تمرینات آبی در سالمندان می‌تواند نقش مهمی در کاهش نوسانات مرکز ثقل آزمودنی‌ها داشته باشد؛ برای مثال رضازاده و همکاران و همچنین باقری و همکاران طی تحقیقاتی 
گزارش کردند مشارکت در اجرای تمرینات آبی می‌تواند عملکرد عصبی عضلانی و تعادل مردان سالمند را تحت تأثیر قرار دهد و احتمال افتادن و زمین‌خوردن را در این افراد به شکل معنی‌داری کاهش دهد [20 ،5]. واکر (2016) و ماتیاس (2013) نیز طی تحقیقاتی به نتایج مشابه با تحقیق حاضر دست یافتند [22 ،21]. 
تحقیق حاضر همچنین نشان داد پس از مشارکت سالمندان در اجرای هشت هفته تمرینات آبی، میزان خطای حس عمقی در مفصل زانوی این افراد به شکل معنی‌داری کاهش یافته است (0/05>P)؛ این در حالی است که با وجود کاهش خطای حس عمقی در مچ پای آزمودنی‌ها پس از شرکت در تمرینات آبی، میزان تغییرات صورت‌گرفته در این مفصل به اندازه‌ای نبوده است که بتواند بهبود معنی‌داری را در دقت حس عمقی آن به همراه داشته باشد (05/0 تحلیل نتایج همچنین حاکی از آن بود که بین میزان تغییرات حس عمقی در مفصل مچ پا و توانایی کنترل پاسچر سالمندان پس از اعمال تمرینات ارتباط معنی‌داری وجود دارد (05/0>P). بر همین اساس به نظر می‌رسد بهبود دقت حس عمقی در مچ پای سالمندان می‌تواند نقش مؤثری در کاهش نوسانات مرکز ثقل این افراد داشته باشد. در مفصل زانوی آزمودنی‌ها نیز مقداری همبستگی بین دقت حس عمقی و توانایی کنترل پاسچر افراد مشاهده شد که البته این میزان همبستگی به اندازه‌ای نبوده است که بتواند ارتباط معنی‌داری را بین متغیرهای مذکور ایجاد کند (0/05

گزارش کردند افزایش دقت حس عمقی در مچ پای آزمودنی‌ها موجب کاهش در میزان نوسانات پاسچر این افراد پس از مشارکت در اجرای چهار هفته تمرینات تعادلی شده است [8]. 
به نظر می‌رسد در تحقیق حاضر یکی از دلایل احتمالی برای معنی‌دار شدن ارتباط بین حس عمقی مچ پا و توانایی کنترل پاسچر آزمودنی‌ها و همچنین معنی‌دارنبودن این ارتباط در مفصل زانو، می‌تواند به نحوه ایجاد اغتشاشات پاسچر و دیستال‌بودن مفصل مچ پا نسبت به زانو مرتبط باشد [23]؛ گزارش‌های پیشین در این زمینه حاکی از آن است که اتصال پاها به زمین و ایجاد زنجیره حرکتی بسته در زمان حفظ تعادل موجب می‌شود مفصل مچ پا اولین مکانیزم جبرانی برای کنترل جابه‌جایی‌های مرکز ثقل در نظر گرفته شود [24]. بر همین اساس به نظر می‌رسد بهبود دقت حس عمقی مچ پا می‌تواند موجب تشخیص دقیق و به‌موقع اغتشاشات اعمال‌شده به بدن شود و با تنظیم مناسب انقباضات عضلانی در اطراف مچ پا، میزان نوسانات مرکز ثقل را به شکل مؤثرتری کنترل کند و موجب کاهش معنی‌داری در شاخص‌های نوسان پاسچر سالمندان شود [25]. 
در خصوص تغییرات حس عمقی مفصل زانو در تحقیق حاضر، یافته‌ها با اکثر گزارش‌های پیشین در این زمینه همخوانی دارد.باقری و همکاران طی تحقیقی گزارش کردند اجرای هشت هفته تمرینات آبی می‌تواند دقت حس عمقی زانو را به شکل معنی‌داری در سالمندان بهبود بخشد [5]. ازدیمیرو همکاران نیز طی تحقیقی گزارش کردند مشارکت در اجرای هشت هفته تمرینات بدنی می تواند بهبود معنی‌دار حس عمقی در هر دو زانوی برتر و غیربرتر آزمودنی‌ها را به همراه داشته باشد [26]. در تحقیق دیگری صادقی و همکاران نیز به نتایج مشابه با ازدیمیر دست یافتند [27]. 
در ارتباط با دلایل احتمالی برای بهبود دقت حس عمقی پس از مشارکت در تمرینات، اشتون میلر و همکاران  معتقدند از آنجا که هیچ تحقیق اثبات‌شده‌ای وجود ندارد که نشان دهد تمرین‌درمانی موجب تغییر در تعداد گیرنده‌های محیطی آزمودنی‌ها شده است، برای توضیح این سؤال که تمرین چگونه موجب تغییر در حس عمقی می‌شود، باید به دنبال مکانیزم‌های مرکزی احتمالی باشیم [28]. در همین راستا گزارش شده است افزایش توجه را می‌توان مکانیزم احتمالی برای بهبود حس عمقی در اثر تمرین در نظر گرفت. توجه فرایندی نوروسایکولوژیک است که سیستم عصبی مرکزی از طریق آن بر اطلاعات دریافتی ثأثیر می گذارد [29]. 
از این رو به نظر می رسد که تمرینات آبی استفاده‌شده در تحقیق حاضر احتمالاً موجب افزایش توجه به علائم حس عمقی در هر دو سطح هوشیارانه و خودکار شده و زمینه را برای بهبود حس عمقی در آزمودنی‌ها فراهم کرده است. مکانیزم احتمالی دیگر برای توجیه بهبود حس عمقی در اثر تمرینات می‌تواند شامل فعال‌شدن مسیرهای عصبی، افزایش تعداد سیناپس‌ها و افزایش منطقه حسی مربوطه باشد که در پلاستیسیته دیده می‌شود. همچنین مطالعات نشان داده‌اند خروجی دوک‌های عضلانی در اثر تمرین افزایش پیدا می‌کند که ممکن است با ایجاد تغییر در تونوس عضلانی موجب افزایش دقت در انجام حرکات شود [28]. 
در ارتباط با تغییرات حس عمقی مچ پا در تحقیق حاضر به نظر می‌رسد نتایج تحقیق حاضر با اکثر گزارش‌های صورت‌گرفته در این زمینه تناقض دارد. تحقیقات پیشین عمدتاً حاکی از تأثیر معنی‌دار تمرینات بر دقت حس عمقی مچ پا هستند؛ چانگ و همکاران گزارش کردند مشارکت در اجرای تمرینات بدنی منظم می‌تواند موجب کاهش معنی‌داری در خطای مفصلی مچ پا شود [30]؛ صادقی و همکاران نیز طی تحقیقی به نتایج مشابه با چانگ دست یافتند [31]. 
اغلب گزارش‌های موجود در این زمینه به صورت تمرینات خشکی انجام گرفته‌اند و با توجه به بررسی‌های صورت‌گرفته، پژوهشی که بهبود حس عمقی مچ پا را بعد از اعمال تمرینات آبی گزارش کرده باشد، یافت نشد. بر همین اساس تفاوت در نتایج به‌دست‌آمده از بررسی‌ها را می‌توان به تفاوت در نوع تمرینات استفاده‌شده در تحقیق حاضر مرتبط دانست؛ به عبارتی به نظر می‌رسد استفاده از تمرینات آبی به کاهش فشار از روی مفصل مچ پا منجر می‌شود و تا حدودی تحریک گیرنده‌های این مفصل را در مقایسه با تمرینات خشکی کاهش داده است [32]. همین موضوع به معنی‌دار نبودن تغییرات در حس عمقی مچ پا منجر شده است.
درباره دلایل احتمالی دیگر که ممکن است در تغییرات معنی‌دار حس عمقی در مفصل زانو و عدم‌تغییرات معنی دار این حس در مچ پای سالمندان نقش داشته باشد، می‌توان به حجم بزرگ‌تری که عضلات اطراف زانو اعم از چهارسر یا همسترینگ در مقایسه با عضلات اطراف مچ پا دارند‌، اشاره کرد. بدین ترتیب عضلات مذکور با ایجاد سطح تماس وسیع‌تری با آب، در مقایسه با عضلات ساق و مچ پا، به تحریک به‌مراتب بیشتری در گیرنده‌های پوستی منجر شده‌اند و متعاقباً بهبود عملکرد آوران‌های حس عمقی را به شکل مؤثرتری در مفصل زانو به همراه داشته‌اند [33]. 
نتیجه‌گیری نهایی
با توجه به نتایج به‌دست‌آمده از تحقیق حاضر، به نظر می‌رسد افزایش دقت حس عمقی در مفصل مچ پا نقش مؤثرتری در بهبود توانایی کنترل پاسچر سالمندان در مقایسه با مفصل زانو دارد. توصیه می‌شود در برنامه‌ریزی‌های تمرینی برای بهبود تعادل سالمندان، به نقش ویژه مفصل مچ پا در بهبود این توانایی بیشتر توجه شود.
