داروهای مناسب تجویزنشده در بیماران سالمند بستری در بیمارستانهای منتخب دانشگاه علوم پزشکی ایران با استفاده از ابزار غربالگری داروهای مناسب تجویزنشده در افراد سالمند

<p style="text-align: justify;"><strong>اهداف:</strong> افزایش جمعیت سالمندان تشویشی جهانی است و مراقبت از سلامت و کیفیت زندگی سالمندان بالای 65 سال، از مشکلات عمده امروز و آینده بهشمار میآید. تجویز دارو جزئی اساسی از مراقبت بیماران سالمند و بهینهسازی درمان دارویی برای بیماران سالمند مسئلهساز است. این امر میتواند عوارض جانبی و هزینهها را افزایش دهد. هدف از انجام این مطالعه بررسی وضعیت داروهای مناسب تجویزنشده در بیماران سالمند60 ساله و بالاتر با استفاده از ابزار غربالگری داروهای مناسب تجویزنشده در افراد سالمند (START) بود که در بیمارستانهای آموزشی و غیرآموزشی دانشگاه علوم پزشکی ایران بستری شده بودند. <br>
<strong>مواد و روشها:</strong> این مطالعه مقطعی در چهار بیمارستان آموزشی و غیرآموزشی دانشگاه علوم پزشکی ایران بهمنظور بررسی میزان داروهای مناسب تجویزنشده براساس معیار START انجام شد. دادههای پژوهش حاضر از پرونده 400 بیمار 60 ساله و بالاتر از بخشهای بستری مشترک (سی.سی.یو، ایی.ان.تی و آی.سی.یو عمومی، ارتوپدی ، جراحی عمومی و داخلی) در طی ششماه (از تیرماه تا آذرماه) سال 1392 جمعآوری شد که بهصورت تصادفی انتخاب شده بودند. تجزیهوتحلیل دادهها با استفاده از نسخه 18 نرمافزار SPSS و نسخه 2013 نرمافزار اکسل انجام شد. همچنین برای تعیین ارتباط بین متغیرهای بررسیشده آزمون کای اسکوئر، من-ویتنی، ضریب همبستگی اسپیرمن و کلموگروف-اسمیرنف بهکاررفت. <br>
<strong>یافتهها:</strong> میانگین سن سالمندان مطالعهشده 73/4 سال بود. از بین سالمندان بررسیشده، بیماری قلبی-عروقی بیشترین درصد (21/6درصد) و بیماری دیابت کمترین درصد فراوانی (3/0) را در بین بیماریهای فعلی سالمندان کسب کرده بودند. تعداد کل داروهای تجویزشده 4744 بود که برای 39/6درصد از سالمندان حداقل یک داروی مناسب تجویزنشده بود. بیشترین طبقات دارویی مناسب تجویزنشده براساس معیار START بهترتیب در بیمارستانهای غیرآموزشی 11/7درصد بیسفسفوناتس و 8/3درصد آنتیدیابتیک و در بیمارستانهای آموزشی 13/3درصد بیسفسفوناتس و 11/4درصد آنتیدیابتیک بوده است. میانگین داروی مناسب تجویزنشده رابطه معنیداری با طول مدت اقامت بیماران در بیمارستانهای آموزشی داشته است. تعداد داروهای مناسب تجویزنشده با افزایش تعداد داروهای تجویزشده بهصورت معنیداری افزایش یافته است.<br>
<strong>نتیجهگیری:</strong> با توجه به اینکه مسائل مربوط به میزان داروهای مناسب تجویزنشده در میان سالمندان بررسیشده میتواند جدی باشد؛ بنابراین تجویز داروهای مناسب برای سالمندان از اهمیت زیادی برخودار است. نظارت بر تجویز، نحوه تأمین و مصرف منطقی دارو بسیار حیاتی و اثرگذار است. ازآنجاکه بخشی از بودجه سلامت به دارو تعلق دارد، بیمارستانها میتواند با بهرهگیری از پزشکان شاغل بههمراه دیدگاههای تخصصی مشاوران دارویی و داروسازان بالینی در کاهش این مشکل کارساز باشد.</p>

<p style="text-align: justify;"><strong>Objectives: </strong>Medication is an essential component of care for elderly patients, and optimizing their process of treatment is often a challenging task. The aim of this study was to evaluate the status of appropriate non-prescribed drugs in 60 elderly patients hospitalized in the teaching and non-teaching hospitals in the University of Medical Sciences employing the START screening tool.<br>
<strong>Methods & Materials:</strong> This cross-sectional study included 400 elderly patients who were randomly selected from various divisions of the hospital (CCU, ENT, ICU General, Orthopedics, Surgery General, and Internal) and were hospitalized within six months duration (from June to December 2013). The data of the selected patients were collected from the hospital records in order to determine the appropriate medications prescribed based on the criterion START. The data was analyzed- using software Excel 2013 and SPSS 18, and the relationship between the variables was determined using the chi-square test, Mann-Whitney, and Spearman correlation coefficient.<br>
<strong>Results: </strong>The average age of the elderly patients was reported to be 73.4 years. Cardiovascular disease had the largest prevalence (21.6%) and diabetes (3.0) had the lowest prevalence among the elderly patients. The total number of drugs prescribed for patients was 4744, of which 158 patients (39.6%) were not prescribed with at least one appropriate medication. In non-training and training hospitals, the most appropriate medications that were not prescribed according to START were related to the bisphosphonates (11.7% vs. 13.3%) and anti-diabetic (8.3% vs. 11.4%) categories. A significant relationship was noted between the average numbers of non-prescribed appropriate medications and the length of stay of patients in teaching hospitals. <br>
<strong>Conclusion:</strong> Since issues relating to the appropriate medications prescribed among the elderly can have serious implications, the drugs prescribed for the elderly are very important. Supervising the prescription, supply, and rational use of drugs is very vital and effective and acts as an integral part of the health budget allocated to the drug. Therefore, hospitals need to readily consult with the trained physicians and consider the expert opinions of clinical pharmacists in handling this problem.</p>
