بررسی فراوانی عوامل خطرآفرین سقوط در سالمندان گیرنده خدمات درمانی در منزل در سال 1396

<div style="text-align: justify;"><strong>اهداف</strong> سالمندان در معرض بیشترین خطر سقوط و زمینخوردن قرار دارند. به منظور طراحی و اجرای مداخلههای مؤثر و کاهش صدمات ناشی از زمینخوردن در سالمندان، لازم است اهمیت نسبی هریک از عوامل خطرزا معلوم شود. این مطالعه با هدف تعیین فراوانی اثرگذاری هریک از عوامل خطر سقوط در میان سالمندان دریافتکننده خدمت از یکی از مراکز ارائه خدمات درمانی در منزل در شهر تهران انجام شد.<br>
<strong>مواد و روش ها</strong> این مطالعه از نوع توصیفیتحلیلی و به صورت مقطعی انجام شد. در این مطالعه 400 نفر از سالمندان، از طریق نمونهگیری طبقهبندیشده و به صورت تصادفی از 22 منطقه کلانشهر تهران انتخاب شدند. از طریق پرسشنامه محققساخته، اطلاعات مرتبط با خطر سقوط در جمعیت هدف، جمع آوری شد. پرسشنامه دو بخش سؤالات جمعیتشناختی و سؤالات مربوط به متغیرهای پژوهش در هفت بُعد (شامل 60 سؤال) داشت. دادهها با نسخه 19 نرمافزار SPSS و با آزمونهای آماری توصیفی و استنباطی مناسب آنالیز و تجزیه و تحلیل شد.<br>
<strong>یافته ها</strong> از مجموع شرکتکنندگان در این مطالعه (004n=)، 5/52 درصد زن بودند و میانگین و انحراف معیار سنی سالمندان مورد مطالعه 8±78 بود. فراوانی سقوط در سالمندان نمونهگیریشده 28 درصد (112 مورد از 400 مورد) بود. از بین سالمندانی که زمین خورده بودند (112n=) 64 نفر(1/57 درصد) زن و میانگین و انحراف معیار سنی آنها 5/7±80 بود. از این بین 82 درصد در داخل منزل با سقوط مواجه شده بودند و محلهایی از منزل که بیشترین فراوانیهای سقوط در آنها رخ داده بود، شامل حمام (6/19 درصد)، سرویس بهداشتی (18 درصد)، اتاق خواب (18 درصد) و پذیرایی (16 درصد) بود. از نظر ناحیه بدن، در ناحیه لگن (8/26 درصد)، دست (3/22 درصد) و پا (6/19 درصد) بیشترین آسیبدیدگی ملاحظه شد. درمجموع 8/43 درصد از زمینخوردن در سالمندان به بستریشدن در بیمارستان با طول دورههای مختلف و درصدهای متفاوت منجر شد؛ به این صورت که 9/44 درصد بین 1 تا 5 روز، 7/36 درصد بین 6 تا 10 روز و 4/18 درصد بین 11 تا 15 روز در بیمارستان بستری شده بودند. رابطه معنادار آماری بین سقوط و هیچیک از عوامل خطر پزشکی، سبک زندگی و محیطی مشاهده نشد.<br>
<strong>نتیجه گیری</strong> درصد زیادی از سالمندان که دچار سقوط شده بودند در بیمارستان بستری شده بودند و درصد زیادی از آنها مدت زیادی را بستری بودهاند که باعث تحمیل هزینههای زیادی بر نظام سلامت شده است. علاوه بر این عوامل، خطر حسیحرکتی مؤثرترین عامل زمینخوردن سالمندان بود که انجام اقدامات بازتوانی و توانبخشی برای سالمندان میتواند باعث کاهش این خطرات شود. این مطالعه نشان داد افرادی که ترس از زمینخوردن دارند بیشتر سقوط کردهاند. بنابراین، در میان عوامل خطر روانشناختی باید به این مورد توجه ویژهای شود.</div>
<div style="text-align: justify;"><strong>Objectives</strong> Elderly people are at the highest risk for falling. In order to design and implement effective interventions and reduce the incidence of falling and its resulting injuries, the relative importance of each risk factor should be determined. This study aimed to determine how frequently different factors contribute to falls among the elderly served by one home health agency in Tehran. <br>
<strong>Methods & Materials</strong> This study was a descriptive-analytic and cross-sectional study. A total of 400 elderly were selected through random cluster sampling method from all 22 districts of Tehran City, Iran. The study data were collected through a researcher-made questionnaire of falling risk factors. The questionnaire consisted of two parts: first, demographic questions and second, questions related to falling in 7 dimensions (including 60 questions). The obtained data were analyzed in SPSS (V. 19) through descriptive and inferential statistics.<br>
<strong>Results</strong> In this study, female samples were 52.5% of the participants (n=400) and the elderly mean±SD age was 78±8 years. The total frequency of falling among the elderly was 28% (112 out of 400). Among the elderly who had fallen (n= 112), 64 (57.1%) of them were female and their Mean±SD age was 80±7.5 years. About 82% of them fell down inside their homes, with these separate frequencies: bathroom (19.6%), toilet (18%), bedroom (18%), and hall (16%). Parts of the body which were more frequently affected by falls in forms of injuries, bone fractures, dislocations, etc., were the pelvis (26.8%), arms (22.3%) and legs (19.6%). Around 43.8% of the falling among the elderly had led to hospitalization, 44.9%, between 1 and 5 days, 36.7% between 6 and 10 days, and 18.4% between 11 and 15 days. The relationship between falls and the medical, lifestyle, or environmental risk factors was non-significant.<br>
<strong>Conclusion </strong>A high percentage of affected elderly were hospitalized after the incident, and among them a high percentage stayed in the hospital for a long time imposing considerable costs to the health system. Moreover, sensory and neuromuscular risk factors were the most frequent causes of falls which could be prevented by rehabilitation measures. Results showed that people who are afraid of falling have a higher risk of falling which requires adequate and special attention to this psychological risk factor.</div>
