Objectives This study aimed to develop and implement an online self-management program and evaluate its effect on hope and anxiety in patients with congestive heart failure.
Methods & Materials In this randomized clinical trial, 100 patients with congestive heart failure were selected using a convenience sampling method and randomly assigned to two intervention and control groups. The data collection tools included a sociodemographic checklist (age, gender, marital status, education level, occupation, time of heart failure diagnosis, comorbidities, medications used, monthly income, family history of heart disease, and Underlying disease and Diagnosis time), Eifert’s cardiac anxiety questionnaire (CAQ), and the Miller Hope scale (MHS). The intervention group received the self-management educational program for 6 weeks via WhatsApp based on Kern’s six-stage model. The collected data were analyzed using descriptive and statistical methods in SPSS software, verson 19.
Results The intervention group showed a significant decrease in cardiac anxiety (from 44.00±8.22 to 44.00±7.025) and a significant increase in hope (from 44±18.15 to 44±16.32). There was no significant difference in the mean CAQ and MHS scores between the pre-test and post-test phases in the control group, but a significant difference was observed in the intervention group (P<0.001).  There was a significant difference in the mean CAQ and MHS scores between the control and intervention groups after the intervention, with the intervention group showing lower anxiety (P=0.004) and higher hope (P=0.021)
Conclusion The implementation of the online self-management educational program can improve hope and reduce anxiety in patients with congestive heart failure.
