Introduction: Older adulthood refers to age above 65 years. In Iran, the elderly involve 8/5% of the whole population totaling 4.5 million people. Because of the disorders in the body defense mechanisms, infections occur more frequently in older adults. On the other hand, the initial demonstrations of infections are atypical in this age group. This study performed an epidemiological investigation of infectious diseases in older adults hospitalized in hospitals affiliated with Birjand University of Medical Sciences during 2016.
Methods: This descriptive, analytic study incorporated all the elderly patients (> 65y) hospitalized in the infectious diseases wards of Valiasr Hospital from April to December 2016.  Purposes of the study were explained to them whereby they offered informed consents for participation. A specific form was completed by a trained nurse for each of the patients based on the information in his/her file. The form consisted of five sections: risk factors, clinical manifestations, paraclinical and radiology tests taken, internal and surjical interventions performed, and complications and outcomes. The data were analyzed in SPSS (version 19) using descriptive statistical tests and Chi-square test. The significant level was set at α=0.05.
Results: A total of 227 patients with a mean age of 76.71±7.6 years (range: 65-97 y) participated in the study. Women accounted for 58.1 percent of the participants. Results showed that 8.8% were smokers and 30.8% were drug abusers of whom 82.9% were oral abusers. Further the most common  underlying diseases were : Hyperthension 36/1% - Diabet 11/5% -Hearth diseases 11% and chronic pulmonary  diseases 9/7%
Final diagnosis in patients were in order to : pneumonia 52/4% - urinary tract infections 15% , septicemia  13/7 Influenza9/7% Gasteroenteritis 7/9% - and others1/3- THE most common organisms that cultured were : sta . aurevs from Blood cultur and Ecoli from urine cultur.
Conclusion: The most frequent infectious disease was pulmonary; therefore, it is suggested to observe individualistic health issues to prevent it.
