Objectives Iron-deficiency anemia in older adults is often neglected. It can play an important role in the occurrence of frailty syndrome. This study aimed to determine the prevalence of iron-deficiency anemia and its association with frailty syndrome in older adults from Bushehr, south of Iran.

Results Among participants, 50.51% were women and the rest were men. The prevalence of frailty syndrome was 18.53%, and 58.2% were pre-frail. The prevalence of anemia in frail people was 13.92%, while in non-frail people it was 8.11%. Multiple logistic regression results indicated a significant association between frailty syndrome and the factors of anemia, depression, age, walking speed, and hand grip strength (P<0.05).
Conclusion Given the prevalence of anemia and frailty syndrome in older adults from Bushehr, and the significant association between anemia and frailty syndrome, it is essential to plan for prevention and effective interventions in this regard.
